At TLC Chiropractic we have a wellness centered approach to our patient care.  As a result our intake form is not only comprehensive, but is designed so that we can get an accurate picture of your overall health, as well as your health goals. 

We ask that you please take the time to print and fill the form out completely, and bring it with you to your first appointment.  If you do not have printer access, simply let us know and we will mail you a copy.  If you have questions about the form, please call us before your first visit so that we can help you.
